STUDENT PARTICIPATION WAIVER FORM

Thank you for choosing ThinkBuild Foundry LLC, where students explore robotics, programming, and hands-on innovation. Please complete this form to ensure a safe and smooth experience for your child.

1. ASSUMPTION OF RISK & LIABILITY RELEASE
I, the undersigned parent or legal guardian, understand that participation in ThinkBuild Foundry LLC programs includes activities such as robotics, coding, electronics, and the use of tools and equipment.

I acknowledge these activities involve inherent risks, including but not limited to minor injuries, equipment-related hazards, and electrical components. I voluntarily assume all risks associated with my child’s participation.

I hereby release and hold harmless ThinkBuild Foundry LLC, its owners, staff, contractors, and volunteers from any and all liability, claims, or demands arising from participation, except in cases of gross negligence or willful misconduct.

2. MEDICAL AUTHORIZATION & EMERGENCY CARE
IN THE EVENT OF AN EMERGENCY, I AUTHORIZE THINKBUILD FOUNDRY LLC STAFF TO ACT IN THEIR REASONABLE DISCRETION TO PROTECT MY CHILD’S HEALTH AND SAFETY.
THIS INCLUDES ADMINISTERING BASIC FIRST AID AND ARRANGING TRANSPORTATION TO A HOSPITAL OR MEDICAL FACILITY.

I GIVE FULL PERMISSION FOR THINKBUILD FOUNDRY LLC REPRESENTATIVES TO SEEK AND AUTHORIZE EMERGENCY MEDICAL TREATMENT FOR MY CHILD, INCLUDING TRANSPORTING MY CHILD TO A HOSPITAL.

I understand that reasonable efforts will be made to contact me immediately. I ACCEPT FULL FINANCIAL RESPONSIBILITY FOR ANY MEDICAL CARE PROVIDED.

5. PHOTO & MEDIA RELEASE

6. STUDENT EXPECTATIONS
Students are expected to:
– Follow all safety instructions
– Respect instructors and peers
– Use equipment responsibly

ThinkBuild Foundry LLC reserves the right to dismiss any student for unsafe or disruptive behavior.

7. REFUND & PAYMENT POLICY
– Registration Fees: All registration or enrollment fees are non-refundable.
– Program Tuition: Full refunds are available if cancellation is requested at least 7 days prior to the program start date.
– A 50% refund will be issued for cancellations made within 7 days of the start date.
– No refunds will be issued once the program has begun.
– Missed Classes: No refunds or credits will be given for missed classes due to absence.
– Program Cancellation: If ThinkBuild Foundry LLC cancels a program, a full refund or credit will be provided.

8. LATE PICKUP POLICY
Students must be picked up promptly at the scheduled end time.

A grace period of 10 minutes is provided. After the grace period, a late fee of $1 per minute will be charged. Late fees must be paid before the student may attend the next session. Repeated late pickups may result in dismissal from the program.
